Clinical Social Worker Responsibilities: Areas include: Patient centered clinical case management, to support health and health related outcomes for individuals with complex health & social needs. Community based case management and field based visits with members occur in a variety of settings and may include private apartments, motels, correctional facilities, encampments, and shelters. Individual reports to Central Office Administration and works closely with an array of partners and providers in the health and social services arena, including state partners. Service delivery using a team based care approach, patient engagement tools, care teams, shared care plans, a strength based approach and motivational interviewing skills. Duties involve need for effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and a number of health care disciplines and service providers; and operational integration with various other health care reform initiatives. Clinical experience and knowledge, knowledge of community, and community culture are necessary.
