About The Position

The Community Resources Specialist is responsible for coordinating a variety of internal and community resources to assist SFHC patients and individuals/families served. This position is responsible for assisting with the completion of various applications, including but not limited to community resources (transportation, housing, food, and/or medication), and various other programs.

Requirements

  • Ability to establish and maintain effective working relationships with staff, patients, and the public.
  • Knowledge of HIPAA privacy rules and regulations.
  • Embraces electronic health records.
  • Excellent interpersonal, verbal, and written communication skills.
  • Strong attention to detail.
  • Ability to manage multiple tasks, work under pressure, and prioritize workload.
  • Ability to follow written policies and procedures.
  • Effective and efficient time management.
  • Ability to work independently and use good judgement.
  • High school diploma required;
  • Computer literacy with knowledge of Microsoft Office (Word, Excel, Outlook);
  • Social and cultural sensitivity appropriate to ethnically and economically diverse participant- and employee-base;
  • Must maintain current Kansas driver’s license, and proof of appropriate auto insurance.
  • English required.

Nice To Haves

  • One or more years of healthcare experience preferred;
  • Bilingual Spanish/English with the ability to speak and read both languages fluently preferred but not required

Responsibilities

  • Provide positive first impression through words and tone to patients, visitors, employees, and community partners.
  • Assist in managing the Manufacturer Patient Assistance Program (MAP), including maintaining patient profiles, applications, provider signatures as well as written and verbal communication with pharmaceutical companies.
  • Communicate in an effective and timely manner with clinic staff, providers and quality staff.
  • Meet with patients when necessary for coordination and maintenance of assistance programs.
  • Assist in the development and implementation of organizational care coordination, quality improvement initiatives, outreach, and community resource initiatives.
  • Coordinate care by serving as the advocate and resource for the patient, their family and their physician, building effective relationships within the community and across the continuum of care.
  • Consult with community partners to provide support services and resources.
  • Keep comprehensive records of clients’ progress throughout the process, including documentation of phone calls, office visits, and referrals.
  • Work collaboratively with and clinical team to promote continuity of patient care.
  • Maintain working knowledge of local resources.
  • Complete all job functions observing confidentiality and HIPAA requirements.
  • Perform other duties as assigned.
