About The Position

The Complex Case Manager (CCM) provides people experiencing homelessness with complex high acuity needs or chronic homeless designation with nuanced case management engagement for solutions to housing placement.

Requirements

  • Bachelor’s degree in social work-related field OR two years of related work experience required.
  • A combination of education and experience may be substituted.
  • Active Driver’s License and insurable under Denver Rescue Mission’s standards to travel to various DRM facilities, partner organizations, and housing properties.
  • Ability to obtain and maintain CPR certification within 90 days of hire.
  • Must be a Christian holding to the beliefs set forth in the Denver Rescue Mission Statement of Faith, expressed by a personal testimony and Christian conduct.
  • Excellent verbal and written communication skills, and responsiveness to communication.
  • Ability to resolve conflict professionally and respectfully while maintaining composure under pressure.
  • Strong working knowledge of Windows software applications and ability to learn SCRIBE and Clarity systems.
  • Ability to follow highly detailed workflows.
  • Ability to professionally engage with clients that are experiencing mental, physical, behavioral and hygienic issues.
  • Time management and organizational skills, being able to independently prioritize the order of tasks and their completion.
  • High level of “team integrated work” requiring strong partnership mentality.
  • Must be willing to drive clients to appointments.

Nice To Haves

  • Colorado Certified Addiction Specialist (CAS) certification or equivalent preferred.

Responsibilities

  • Maintains the relationships necessary in community and across DRM to ensure DRM’s case management efforts reflect a real-time understanding of broader re-housing efforts, availability, requirements, and protocols.
  • Demonstrates respect, cultural competence, and knowledge of trauma-informed care in interactions with clients, staff, and other community providers.
  • Develops a case plan with individualized goals based on individual acute needs.
  • Identifies action steps necessary to achieve goals and monitors progress through scheduled follow up meetings.
  • Schedule will include some early mornings and/or late evenings.
  • Maintains professional best practices in areas of document storage, confidentiality and professional ethics when dealing with clients and client information
  • Regularly updates all case planning records, services, and case notes in SCRIBE including uploading vital documents and ROIs.
  • Documents all meetings, referrals, and services in SCRIBE utilizing the appropriate templates.
  • Provides SCRIBE reports, spreadsheets and statistics as requested.
  • Engages in ongoing training as assigned by direct supervisor and leadership team.
  • Seeks to develop a continuing knowledge of the field, what services are provided in the community, and establishes communication with those entities.
  • Collaborates with other DRM personnel regarding client evaluation, goals, and continuity of care.
  • Facilitates community provider appointments with clients including case note documentation in SCRIBE (Denver Human Services, Medication Assisted Treatment, Veterans Administration, Social Security Income/Social Security Disability Income Attorney, Rocky Mountain Human Services, Colorado Access).
  • Collaborates, communicates, and advocates with service providers in the government, medical, mental health, housing, benefits acquisition, employment, legal, and veterans, etc. fields to aid clients toward a successful transition into long-term programs or sustainable housing.
  • Provides housing focused case management to the most complex cases in our city with the goal of connecting clients to long term solutions, paying particular attention to coordination of services and care as well as acquiring vital documents, benefits, and providing housing navigation.
  • Completes documentation such as case notes, service recording, entries into HMIS/Scribe within 48 hours of client contact.
  • Responds to community collaboration such as emails, phone calls and appointments thoroughly, professionally, and within three business days.
  • Transports individuals to appointments that will aid in stability and housing acquisition using company vehicle.
  • Services clients both inside DRM properties and out in the community at appointments, outdoor areas, and public places.
  • Attends staff meetings and is prepared for case manager and team meetings to address critical needs and provide creative solutions for engagement and resolution.
  • Provides high level of attention to detail in schedule, community connection, documentation while maintaining creativity and flexibility required to establish progress with a high need caseload.
  • Other duties as assigned.
