The RN - Complex Care Case Management shall be responsible for a specific population of patients that are considered complex and difficult to discharge from the hospital setting. The RN will manage the discharge during post-acute care until the patient is able to transition. This individual is responsible to manage these patients’ successful transition from hospital through the intermittent post-acute care setting to the home, and is accountable to develop, implement, and evaluate comprehensive transitional care interventions for high risk medical, surgical, and/or trauma patients. Additionally, this individual will coordinate and oversee the ongoing program metrics to continually improve care for the complex patient population. This position may have additional duties assigned that are within scope of the role.
