Panel management is an organized, population-based, data-driven multi-disciplinary team approach, in which the primary care teams manage patient care using registry tools to identify patients’ unmet care needs, gather summary information for care interventions, and communicate with patients. As an extension of the patient-centered medical home, the Panel Coordinator identifies patients in need of preventive and chronic disease management services, performs outreach through multiple methods, and tracks improvements in population metrics. The Panel coordinator has accountability for monitoring and presenting quality scores and performance metrics and participating in quality initiatives that impact performance. This role will have an assigned primary location but may be required to work at or travel to different locations as needed.
