The Patient Outreach Coordinator II provides administrative, operational, and patient support activities for the Trinity Health Medical Group (THMG) Quality Team, practices, and providers. This includes but is not limited to outreach to patients with gaps in chronic disease management and preventive services, attribution correction and provider panel clean up, and chart audits to workflow. Supports and promote the success of population health programs and care coordination activities. Provides support and mentoring to Panel Coordinator I and leads patient outreach coordinator special projects.
