POSITION SUMMARY Overseen by Senior Manager of HCBS Programs, the HCBS Provider is responsible for providing a variety of community-based non-clinical rehabilitation services to individuals. The HCBS Provider assesses the individual’s need to develop a specific, measurable, and attainable plan to achieve his/her goal(s). HCBS Provider will work as part of an interdisciplinary team within HCBS.

Requirements

  • Bachelor’s degree in mental health or social services related field required
  • Communication, interpersonal, and organizational skills
  • Valid driver’s license and meets all the criteria to be a valid driver for the agency.
  • Willingness to travel a must. Services provided throughout Nassau and Suffolk County on a case by case basis.

Nice To Haves

  • One year of experience providing individual, group, or family therapy to individuals with mental illness is preferred

Responsibilities

  • Provides non-clinical, rehabilitative services
  • Effectively communicates and supports the organization’s mission, vision, and values
  • Assists Core Clinicians with a caseload of approximately 20 individuals
  • Communicates observations regarding goal progress to Supervisor
  • Ensures documentation meets quality and compliance thresholds as set by regulation and/or Agency guidelines
  • Utilizes electronic medical record to complete required documentation as required by regulation and within mandated time frames
  • Provides field-based non-clinical treatment in the community and individual’s home as required
  • Adheres to the organizations Code of Ethics
  • Always communicates in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community
  • Completes all required trainings
  • Maintains confidentiality
  • Maintains and contributes to client caseload including but not limited to non-clinical treatment planning recommendations, drafting progress notes, and assisting with coordinating/documenting discharges.
  • Prevents, recognizes and reports abuse toward the people we support
  • Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.
  • Performs other duties as assigned.
  • Always communicates in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
  • Complies fully with FREE’s confidentiality policies by holding in strictest confidence all information pertaining to members, residents, the people we support and their families, clients, providers, contractors, and employees.
  • Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk.
  • Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours.
  • Meets all job requirements and stays in compliance.
  • Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations.
