About The Position

The Diabetes Care and Education Specialist (DCES) deliver comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardiometabolic care. The DCES is an integral part of the behavior change and improved quality of life across the lifespan. The DCES supports and advocates for people affected by diabetes to optimize quality care. The DCES promotes self-management to achieve individualized behavioral and treatment goals that reduce the risks and optimize health outcomes.

Requirements

  • Must hold an accepted degree in the Allied Health Field and be currently licensed (Unencumbered) in the state of Montana. Options for SDPI Grant Approved allied health licenses include B.S. Nursing, Registered Dietician, APRN. Prior authorization by the Funding Agency will be required for applicants who do not hold a degree in the allied health field.
  • Valid license based on location of practice as a Dietitian (must be registered by the Commission of Dietetic Registration), or Registered Nursed, or Registered Pharmacist.
  • Basic Grantsmanship
  • Knowledge of the Rocky Boy Reservation as to its geographies to population groups, health needs, and cultural differences.
  • BLS certification (may be obtained within the first 6 months of hire)

Nice To Haves

  • Must hold an accepted degree in the Bachelor of Science degree in Allied Health Field or related field and active CNA Certification.
  • Experience in the field of health care
  • Knowledge of and ability to address and apply the latest EBP guidelines to collaboratively to ensure positive patient and community health outcomes.
  • Knowledge of quality assurance principles as they relate to program requirements.
  • Ability to assess large amounts of data correctly and disseminate that data appropriately to key stakeholders.
  • Bachelor of Science Degree required.
  • High level of knowledge related to Diabetes Care Management including disease process, prevention, nutrition, SDOH, lifestyle modification techniques.
  • Previous Grant Management and Budget Experience
  • Ability to generate and interpret reports through iCare, RPMS Diabetes Management system (DMS) and the Electronic Health System (E.H.R.).

Responsibilities

  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions and self-management of diabetes into clinical practice.
  • Advocates for and communicates about improved quality of care and oucomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions.
  • Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.
  • Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardiometabolic conditions across the lifespan.
  • Will maintain 15 hours of continuing education annually at it relates to the profession.
  • Contributes to research and applies current research and evidence-based care to practice.
  • Apply business principles, systems practice, and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers.)
  • Applies the AADE7 self-care behaviors to educate and initiate behavior change.
  • Engages in lifelong learning and serves as a role model of professionalism
  • Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines
  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient assessed needs and goals and promotes successful self-management.
  • Documents all individual contacts/visits in the electronic Health Record and outcomes databases according to the guidelines in a timely manner.
  • Collaborates, advocates and confers other members of the diabetes care team in developing person-centered diabetes plans.
  • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services
  • Collaborates with community partners to establish and maintain on-going support options.
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
  • Perform comprehensive Foot Exams and Routine footcare for PWD and collaborate with providers for follow up care.
  • Maintains appropriate record keeping for participants in structured diabetes education programs for submitting to ADCES/DEAP.
  • Works with the DM Staff to coordinate health care needs of participants through assessment, evaluation, and referral to appropriate resources.
  • Tracking diabetic program participants. Will send letters generated from the DMS to diabetic patients who have follow up needs o a quarterly basis.
  • Assists in maintaining an official accreditation binder for Audit purposes. Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate
  • Undertake additional responsibilities as delegated and directed by the supervisor to support the overall functioning and objective of the Rocky Boy Health Center, which may include tasks such as assisting with administrative duties, participating in quality improvement initiatives, contributing to team projects, or engaging in professional development activities.
