Requirements

Bachelor’s Degree in nutrition or related subjects, preferred
Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance
Two years of experience as a Dietician in a clinical care setting, hospice or home health preferred.
Ability to establish and maintain effective working relationships with all segments of the staff, the lay and professional public, the Board of Directors and Medical Director.
Understands hospice philosophy, focus of care is comfort vs. rehab and to maintain care in their residence as long as possible

Responsibilities

Provide direct dietary assessment of patient
Ability to establish plan of care to meet the dietary needs of the patient
Provide education to patient and caregiver regarding diet management
Appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
Provide counseling and education to IDG, nursing staff and physician, as appropriate
Timely submission of all documentation
Performance of other duties as required
Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
