About The Position

JOB SUMMARY: Provides consultation to the health care team to ensure efficient and effective nutrition care of patients. Participates in ongoing process improvement activities within the Department of Food and Nutrition Services and the center. Functions as expert in the area of clinical nutrition and maintains a customer- service, patient-centered approach.

Requirements

  • Registered by the Commission on Dietetic Registration of the American Dietetic Association in good standing.
  • Completed an internship or practical work experience that includes rotation in long term care.
  • Ability to communicate and deal tactfully with employees, residents, families, visitors, and others.
  • Demonstrated excellence in leadership skills, customer service and nutrition clinical skills.
  • Ability to work flexible hours and alternative schedules, if needed.
  • Ability to stand, bend, stoop, and stretch for extended periods of time.
  • Frequently uses manual dexterity to finger, handle, feel, or manipulate equipment.
  • Ability to talk, listen, hear, and communicate with patients, residents, staff, and visitors.
  • Ability to maneuver throughout the department, including small or narrow spaces.
  • Ability to push, pull, move, or lift a minimum of 25 pounds.

Responsibilities

  • Provides consultation to the Director of Dietary Services and the Administrator on federal, state, and local regulations pertaining to food service operations and nutrition care.
  • Assesses patients’ nutritional needs and monitors and evaluates patient care on an ongoing basis.
  • Provides education to patients, health care team members and members of the community.
  • Assesses the resident’s nutritional status, diagnose nutrition problems and risks, and develop interventions to meet needs.
  • Completes or assists in completing the relevant sections of the MDS.
  • Using the Resident Assessment Instrument (MDS) and other approved assessment tools, develops a nutrition care plan that identifies nutrition problems, goals to be met for each problem, and interventions to achieve those goals.
  • Ensures nutrition care plans are updated as the resident’s condition or interventions change.
  • Maintains good communication with all disciplines to ensure a team approach in developing each resident’s plan of care.
  • Completes nutrition documentation in a timely manner.
  • Attends weekly standards of care meetings involving weight and wounds.
  • Monitors meals served for compliance with physician’s orders and menu extensions.
  • Ensures menus are followed and substitutions are appropriate.
  • Participates in and makes recommendations for the Dining Services quality assurance program.
  • Available during State and Federal surveys to provide consultation and guidance unless otherwise directed by the Administrator.
  • Reviews and assists in developing a plan of correction for any dietary or nutrition care deficiencies noted during state, federal or local health department surveys.
  • Completes a monthly sanitation and safety audit of the Dietary Services Department and report results to the Director of Dining Services and the Administrator.
  • Communicates regularly with the Administrator regarding nutrition or dietary concerns.
  • Provides in-service training to facility staff on the nutritional needs of residents or other topics as requested.
  • Participates in facility Manager on Duty program as directed by Administrator.
  • Maintains the confidentiality of all resident care information.
  • Other duties, as assigned
