About The Position

At A&I Avenues, we believe that early intervention changes lives. We're seeking a dedicated and compassionate Service Coordinator to join our Early Intervention team and help families of children ages 0–3 access the essential services they need to thrive. This is a meaningful, people-centered role where your work directly empowers families, connects children to vital supports, and ensures coordinated care from day one. You'll guide families through Early Intervention services, develop Individualized Family Service Plans (IFSPs), and ensure quality, compliance, and compassion in every step of care.

Requirements

  • High School diploma
  • A valid driver’s license and reliable personal transportation
  • Comfort using a personal mobile device for work communication

Nice To Haves

  • Bachelor’s degree in a human services-related field
  • OR 5+ years of work or volunteer experience in disability or early childhood programs
  • OR a combination of education and experience appropriate to the role
  • Master’s in Social Work (MSW)
  • Experience supervising social work interns
  • Familiarity with Medicaid, IFSPs, or early childhood systems

Responsibilities

  • Developing and implementing Individualized Family Service Plans (IFSPs) that reflect each child’s unique strengths, needs, and family priorities.
  • Coordinating and monitoring services and supports to ensure alignment with IFSP goals.
  • Serving as the main point of contact for families, providers, and other team members throughout the early intervention process.
  • Advising families on available service options and assisting with access to needed supports.
  • Leading periodic and annual IFSP reviews, facilitating communication and collaboration among team members.
  • Advocating for families and helping resolve service-related concerns, including guiding them through appeals if necessary.
  • Ensuring timely, accurate documentation in compliance with federal, state, and organizational regulations.
  • Maintaining monthly contact with families and completing Targeted Case Management billing.
  • Supporting transitions or exits from services when appropriate, ensuring families are well-informed and supported.
