We are seeking a Care Manager to support older adults in Butler County by assessing needs, coordinating services, and ensuring quality care through the Elderly Services Program (ESP). In this role, you’ll work directly with clients and caregivers to develop personalized service plans, connect them with resources, and provide ongoing support to enhance their quality of life. Status: Full-time Location: Butler County, Ohio Pay: $18.00/hr and up, based on experience Schedule: Primarily daytime hours with some flexibility as needed Work from home and in field, in office one day week. One year of experience in geriatrics, case management, long-term care, home health, or medical social work preferred. Valid driver’s license, insurance, and reliable transportation for home visits. Acceptable criminal records check required. Strong computer skills, including basic applications and care management software.
