About The Position

Movn Health is looking for care managers to join our team and mission to provide cardiac rehab to 1 million patients every year. As part of this role, you will be responsible for managing patients on a weekly basis to improve risk factors so they can decrease risk of future cardiovascular events. About Movn Health Movn Health provides Movn - a digital cardiac rehabilitation service to help patients recover after a heart attack or heart surgery. The goal of the program is to help patients reduce their risk of getting a future cardiac event by empowering them to manage their risk factors including exercise, nutrition, smoking, blood pressure, weight, medication and stress. We are clinicians, designers, engineers and entrepreneurs leveraging design, research and technology to improve patient engagement and care coordination. Requirements

  • Minimum Bachelor degree in exercise related science or nursing.
  • 1-2 years Past experience in telehealth, remote patient monitoring, chronic care management or virtual care
  • Certifications required:
  • If Exercise Physiologist: Exercise related science: ACSM-CEP preferred or willing to sit for the CEP within 6 months of hire)
  • 1-5 years of prior experience in cardiac education, care management, cardiac rehab and secondary prevention required
  • Pulmonary rehab and PAD experience preferred as well.
  • If Registered Nurse: (preferably with CCRP certification, or willing to sit for the CCRP exam within 6 months of hire)
  • Preferably with a compact RN license or willing to obtain a compact license as well as single state licenses in non participating states.
  • 1-5 years of prior experience in cardiac education, care management, cardiac rehab and secondary prevention required
  • Ability to work 2 pm -6 pm Eastern 3-4 days a week
  • Tech savvy, with a firm grasp on effective and efficient communication through digital platforms such as Slack, Google Meets, Skype, Zoom.
  • Experience executing quality improvement initiatives to lower re-admissions and improve patient outcomes.
  • A compassionate and empathetic “people person” who is able and willing to work independently.

Nice To Haves

  • Someone preferably bilingual (Spanish, Mandarin, other languages a plus)
  • Someone willing to work flexible hours depending on their location.
  • Preference given to candidates who can also work Saturdays.

Responsibilities

  • Guide participants in 1:1 video calls and group education sessions through our 3 month online program to help them manage their cardiac related risk factors.
  • Development of comprehensive treatment plans that is tailored to the individual risk factors and medical needs of the patients.
  • Treatment plans include exercise prescriptions, medications and interventions for smoking cessation, stress and hypertension management.
  • Engage patients weekly to provide coaching, education, emotional support and modify treatment plans through video, voice or text messages.
  • Coordinating the approval of treatment plans remotely with Medical Directors.
  • Provide progress reports on an as needed basis when symptoms are reported or vital signs are not at goal.
  • Share approved treatment plans to referring providers on a monthly basis to provide insight into the patient’s performance in the program.
  • Monitor dashboard for adverse events and triage appropriately.
