About The Position

In this role, success isn't just about paperwork; it's about removing the financial and systemic barriers that stand between a patient and their treatment plan. Champion patient access by identifying barriers to care and conducting comprehensive needs assessments to ensure every patient has a clear roadmap to the services they require. Empower patients and families through personalized education and social support, guiding them through complex medical applications and scheduling to ensure no appointment is missed. Bridge the gap between resources by cultivating deep partnerships with community agencies and internal departments, ensuring a seamless flow of support for those in our care. Advocate for clinical continuity by meticulously documenting every touchpoint and outcome, ensuring a transparent patient history that supports long-term quality of care.

Requirements

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

  • Prior work experience in the medical field preferred.
  • Must be able to demostrate organizational and detail oriented skills.
