About The Position

The Health Home Administrative Assistant provides administrative, data management, compliance, and coordination support to Health Home operations. This role ensures efficient workflows, accurate documentation, timely data entry, and adherence to internal and external policies, supporting operational efficiency and performance monitoring across Skyward Health.

Responsibilities

  • Enter and maintain external referrals and referral lists in external platform, including those from Managed Care Organizations (MCOs) and growth initiatives.
  • Ensure accuracy and completeness of all referral data.
  • Maintain and update MCO contact sheets and tracking tools.
  • Submit HARP Plans of Care and distribute overdue HARP assessment lists.
  • Coordinate scheduling of care conferences with MCOs.
  • Distribute MCO audit findings to CMAs and track follow-up actions.
  • Collaborate with Quality leadership to enhance tracking tools, identify trends, and address reporting or communication issues.
  • Manage administrative components of Health Home audits, including SharePoint folders (public, private, and QMP shared).
  • Track CMA onboarding, Corrective Action Plans (CAPs), and Performance Improvement Project (PIP) attestations.
  • Assist with preparation for external audits and Fair Hearings, including gathering documentation required for follow-up.
  • Monitor compliance with Health Home documentation requirements.
  • Track CMA survey submissions and summarize results for leadership.
  • Manage operational email inboxes and coordinate internal and external meeting invitations.
  • Distribute CMA network communications and ensure timely responses and follow-up needed across the organization.
  • Create and maintain organized systems including documentation, tracking tools, and internal platforms (Teams, SharePoint).
  • Run reports from internal and external systems (e.g., FCM, MAPP, PSYCKES, SharePoint).
  • Assist Executive Director and Senior Leadership with special projects, scheduling, planning, and expense documentation.
  • Support development and maintenance of internal and external Policy & Procedure manuals.
