About The Position

The College of Veterinary Medicine invites applicants to apply for the position of Health Information Clerk, located within the Veterinary Medical Center. The Health Information Clerk ensures patient medical records are properly processed, maintained, and secured in accordance with established policies and legal guidelines. This position focuses on accurate record review, data entry verification, census maintenance, and customer service support.

Requirements

Health Information Clerk 1: Minimum Education: High School Diploma or GED Minimum Level/Type/Years: Entry Level Must be at least18 years of age. Knowledge, Skills, & Abilities: Excellent interpersonal communication and customer service skills. Basic proficiency in Microsoft Office Suite. Ability to use Microsoft Excel for data entry and simple spreadsheet updates. Familiarity with basic formatting, sorting, and filtering functions in Excel. Ability to follow established spreadsheet templates to input and maintain data accurately. Strong computer skills. Ability to effectively solve routine problems. Ability to maintain confidentiality. Ability to work with a diverse population. Strong attention to detail and accuracy. Ability to multitask and work independently with frequent interruptions.
Health Information Clerk 2: Minimum Education: High School Diploma or GED Minimum Level/Type/Years: 1 year of relevant experience. Must be at least18 years of age. Knowledge, Skills, & Abilities: Proficient in Microsoft Office Suite, particularly Microsoft Excel. Ability to create and modify spreadsheets for tracking data and departmental workflows. Working knowledge of Excel functions including sorting, filtering, basic formulas, and data organization. Ability to generate routine reports and track productivity metrics using Excel. Ability to identify minor data discrepancies and correct errors. Strong working knowledge of electronic medical record systems. Demonstrated ability to analyze information and resolve moderately complex issues. Excellent organizational and communication skills.
Health Information Clerk 3: Minimum Education: High School Diploma or GED Minimum Level/Type/Years: 2 years of relevant experience. Must be at least 18 years of age. Knowledge, Skills, & Abilities: Advanced proficiency with EMR systems and data management. Proficient in Microsoft Office Suite, particularly Excel (data entry, formatting, sorting, filtering, basic formulas). Strong computer skills with demonstrated proficiency in Microsoft Excel for data tracking and reporting. Ability to use Excel to maintain spreadsheets, track metrics, and generate routine reports. Demonstrated ability to work independently and exercise sound judgment. Strong analytical, organizational, and problem-solving skills. Ability to mentor and guide junior staff.

Responsibilities

Performs comprehensive review of each patient’s clinical visit for completeness.
Verifies accuracy and validity of data entered into the electronic medical record (EMR).
Identifies documentation deficiencies and works with clinicians to resolve and close consults.
Maintains strict confidentiality of patient and client information.
Assists with routine Release of Information (ROI) requests.
Scans and uploads records from legacy systems into the current EMR.
Processes routine internal record requests.
Maintains the daily patient census, ensuring accuracy of visit status, admissions, discharges, and transfers as applicable.
Updates and reconciles census data within electronic systems and designated tracking spreadsheets.
Answers phones and assists internal and external callers.
Performs other related duties as assigned.
