About The Position

Indiana Masonic Home Compass Park is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Health Team. Compass Park is a beautiful 320-acre senior living community located in the heart of Johnson County, just minutes from Indianapolis. Our campus offers a vibrant environment throughout all seasons, surrounded by tall trees, scenic walking paths, and a welcoming community atmosphere. This position is based in Bloomington/Bedford area

Requirements

  • Graduate of an accredited school of nursing
  • Current Licensed Practical Nurse (LPN) license in the State of Indiana
  • Minimum of one (1) year of experience in an acute care setting or equivalent experience
  • Current CPR certification
  • Strong written, verbal, and interpersonal communication skills
  • Valid driver’s license with a reliable, insured vehicle in accordance with state and agency requirements
  • Ability to complete electronic/paperless documentation

Responsibilities

  • Provide professional nursing care as defined by the Nurse Practice Act and under the direction of a Registered Nurse (RN).
  • Communicate appropriate changes in client status or condition to the RN according to agency guidelines.
  • Provide skilled nursing visits for clients as directed by the RN Case Manager and follow the established plan of care.
  • Communicate information in a timely manner consistent with agency policy and the urgency of the situation through both verbal communication and accurate documentation in the clinical record.
  • Promote personal safety and maintain a safe environment for clients and coworkers by recognizing and responding appropriately to potentially unsafe situations.
  • Make recommendations for safety improvements and seek training or assistance when needed.
  • Perform duties in compliance with agency policies, professional standards, and state and federal regulations.
  • Maintain strict confidentiality regarding all client and staff information and refrain from discussing identities, conditions, or circumstances with unauthorized individuals.
  • Participate in the development, implementation, and evaluation of the agency’s Quality Improvement program and related activities.
  • Assist in coordinating home care services, attend required meetings, and follow appropriate reporting guidelines.
  • Accurately and promptly document client visits and care provided in the clinical record.
  • Recognize and report changes in client condition to the Case Manager and physician as directed.
  • Perform additional duties and responsibilities as assigned.
  • Maintain confidentiality of all resident, employee, and operational data.
  • Support quality assurance initiatives and comply with all regulatory requirements.
  • Maintain compliance with all laws and policies that ensure a workplace free from harassment and discrimination.
  • Support and participate in a team-oriented work environment by cooperating with coworkers and completing job duties with minimal supervision and sound judgment.
