Requirements
- Current, unrestricted Colorado LPN license
- Ability to work independently in a home-based setting
- Strong clinical judgment and communication skills
- Comfort with documentation and basic technology (EMR training provided)
- Valid driver's license, reliable vehicle, and auto insurance
Nice To Haves
- Home health experience preferred (but not required - we provide training)
Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Perform nursing visits including assessments, treatments, and follow-up care
- Administer medications and treatments within LPN scope of practice
- Monitor patient condition and report changes to the supervising RN or care team
- Educate patients and caregivers on medications, treatments, and safety
- Complete accurate, timely documentation in the EMR with training and support provided
- Collaborate with RNs, therapists, caregivers, and leadership to support coordinated care
- Travel locally within Mesa County (mileage and drive time reimbursed)
