Requirements

  • Current, unrestricted Colorado LPN license
  • Ability to work independently in a home-based setting
  • Strong clinical judgment and communication skills
  • Comfort with documentation and basic technology (EMR training provided)
  • Valid driver's license, reliable vehicle, and auto insurance

Nice To Haves

  • Home health experience preferred (but not required - we provide training)

Responsibilities

  • Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
  • Perform nursing visits including assessments, treatments, and follow-up care
  • Administer medications and treatments within LPN scope of practice
  • Monitor patient condition and report changes to the supervising RN or care team
  • Educate patients and caregivers on medications, treatments, and safety
  • Complete accurate, timely documentation in the EMR with training and support provided
  • Collaborate with RNs, therapists, caregivers, and leadership to support coordinated care
  • Travel locally within Mesa County (mileage and drive time reimbursed)
