This role is ideal for an RN who thrives in care coordination, clinical oversight, and interdisciplinary collaboration. You will ensure high-quality, patient-centered home health services while supporting field staff and maintaining regulatory compliance.
Requirements
- Active Maryland RN License (Required)
- Minimum 3 years of clinical experience (Home Health strongly preferred)
- Knowledge of Medicare regulations and home health standards
- Strong communication and leadership skills
Nice To Haves
- Case Management or Care Coordination experience preferred
- EMR experience preferred
Responsibilities
- Oversee and coordinate patient care plans in the home health setting
- Conduct comprehensive assessments and reassessments
- Develop and manage individualized care plans
- Collaborate with physicians and interdisciplinary team members
- Monitor patient outcomes and adjust care plans as needed
- Provide clinical guidance and support to field staff
- Ensure compliance with Medicare, state, and accreditation standards
- Maintain timely and accurate documentation
