About The Position

The Admissions Registered Nurse completes hospice admission assessments for patients in their homes or facility settings and supports a smooth transition into hospice services. Responsibilities include conducting initial visits, evaluating eligibility, educating patients and families, and completing timely documentation to initiate care. Schedule & Coverage • Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM. Requirements

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse, current Drivers License
  • Two years experience as a Registered Nurse in a clinical care setting and 1 yr full time hospice.
  • Nursing skills as defined as generally accepted standards of practice.
  • Knowledge of State and Federal Regulations.
  • Good interpersonal skills.
  • Reliable transportation.
  • Valid and current auto liability insurance.
  • Works in patients home in various conditions and locations; possible exposure to blood and bodily fluids and infectious diseases; Proof of current CPR.
  • Ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.

Responsibilities

  • Complete hospice admission assessments and eligibility determinations
  • Educate patients and families on hospice services, philosophy, and care expectations
  • Coordinate admission processes with clinical and operational teams
  • Obtain physician orders and ensure accurate documentation
  • Document admission findings in the EMR (HCHB)
  • Participate in IDG/IDT communication as needed
