About The Position

The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient-centered hospice that maximizes resources at bedside while being operated locally. Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve. When end-of-life care is necessary, patients and their families are in their most vulnerable time. Our Ohio-based management team, through our local clinicians, can respond to the needs of our patients in a timely and appropriate manner because we are local. We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Dayton, OH area. We dont require high caseloads or unmanageable quotas, which gives our team more time with each of their patients. Our top priority is our patients and the people serving them. Schedule: Monday-Friday 8a-5p

Requirements

  • Bachelors degree in related field
  • Experience with families/caregivers and patients through death/dying issues
  • Strong communication skills and interpersonal skills
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss
  • Valid drivers license and proof of insurance is required

Responsibilities

  • Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family caregivers
  • Plans for Bereavement Program development, expansion and refinement through annual evaluation of services
  • Provides bereavement support to hospice personnel coping with work related grief through on-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources
  • Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed
  • Assists as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups)
  • Functions independently, but seeks and accepts guidance from other members of the team and or members of the community
  • Participates in hospice activities such as in-service education, hospice personnel meetings, and relevant committees
