About The Position

A Housing Care Manager - HF contributes to AHF by supporting clients referred through Housing Forward’s continuum of care, including individuals who are unhoused and seeking permanent housing, as well as those currently housed who require ongoing support to maintain stability. Serving within the Permanent Supportive Housing Scattered-site program, the Housing Care Manager - HF facilitates access to housing and supportive services by identifying client needs, developing individualized service plans, and conducting regular follow-ups to ensure progress and retention. They work with partner agencies to coordinate care, remove barriers to housing, and provide targeted referrals for behavioral health, medical, financial, and social services.
A Housing Care Manager - HF enhances the reputation of AHF by providing consistent, field-based support to residents in the Permanent Supportive Housing Scattered-Site program. Managing a caseload of up to 25 clients, the Housing Care Manager - HF conducts at least two in-home visits per client each month to monitor housing stability, identify emerging needs, and provide timely referrals to appropriate services. They attend regular coordination meetings with Housing Forward and partner agencies to align care strategies and ensure adherence to Housing Forward Continuum standards of care. In addition to direct client support, the Housing Care Manager - HF - works to build relationships with external service providers and identify opportunities for formal partnerships through memorandums of understanding (MOUs). Travel is expected at least 50% to clients’ housing units, area resources, and various agencies required to meet client needs.

The Housing Care Manager - HF will work as a part of a close-knit team to support residents in the Permanent Supportive Housing Scattered-Site program by accepting referrals, completing client enrollments, and developing individualized care plans. They will facilitate access to transportation and provide targeted referrals to behavioral health, medical, and social services, with a strong emphasis on connecting clients to behavioral health support through Housing Forward’s partner agency, NTBHA. The Housing Care Manager - HF will become familiar with Housing Forward’s intake, recertification, and exit processes, and will document all client encounters and actions taken on the client’s behalf. They will also contribute to a dynamic resource database and help ensure services are equitable, inclusive, and tailored to individual needs. Through active coordination with service providers and participation in regular meetings with Housing Forward and partner agencies, the Housing Care Manager - HF helps maintain alignment with continuum standards of care and promotes long-term housing stability.

Requirements

• BS/BA in Social Work, or other social service field with two (2) years of direct service delivery.
• Familiarity with housing case management.
• Familiarity with housing systems and procedures
