About The Position

Under the direction of the Youth Housing Services Manager, the Housing Navigator is responsible for providing housing navigation, outreach, housing placement, and crisis intervention for transitional-age youth (18-24 years old) participating in The Center's LGBTQ Safe STAY Program.

Requirements

  • 2 years of experience providing housing-related services to LGBTQ youth, people living with HIV, or marginalized groups of individuals.
  • Fluency in English required.
  • Must be comfortable working in PC/Windows-based environments.
  • Demonstrated ability to manage multiple tasks with shifting priorities.
  • Must demonstrate competence in working with diverse populations.
  • Must demonstrate competence in using strength-based, trauma-informed, and positive youth development approaches in service delivery.
  • Reliable transportation, a valid driver's license, and auto insurance.

Nice To Haves

  • 1-2 years of experience working with people experiencing homelessness, foster care, and juvenile dependency systems.
  • Fluency in Spanish is highly preferred.
  • Experience managing crises and implementing crisis interventions.

Responsibilities

  • Conduct assessments with clients experiencing housing instability, including Transitional Age Youth Vulnerability Index-Service Prioritization Decision Assistance Tool (TAY VI-SPDAT).
  • Assist clients with applications, eligibility documentation, and other housing-related activities.
  • Provide ongoing support to clients and accompany them to housing appointments as needed.
  • Provide education to clients related to housing, leasing, and tenant rights and responsibilities.
  • Guide clients in accessing external services that support their health and well-being.
  • Maintain accurate documentation of client goals and needs, individualized housing plans, and service entries into internal data systems.
  • Enter program data and outcomes into the region's Homeless Management Information System (HMIS) to adhere to grant and contract compliance.
  • Document thorough case notes that accurately reflect client progress toward individualized housing goals.
  • Assist in the completion of monthly narrative reports for funder reports.
  • Build and maintain relationships with housing providers.
  • Attend housing provider collaborative networking meetings and help identify new housing opportunities for clients.
