About The Position

Under the supervision of the Program Manager and Coordinator, the Intake and Screening Specialist serves as the initial point of contact for individuals and families seeking homeless prevention or homeless services. This position is responsible for conducting screenings, eligibility determinations, and referrals in accordance with contractual regulations and coordinated entry requirements. The Specialist ensures timely access to appropriate services while maintaining accurate documentation and data integrity.

Requirements

  • High school diploma or equivalent required; Associate’s or Bachelor’s degree in human services or related field preferred
  • Experience in intake, screening, customer service, or human services
  • Attention to detail and accuracy
  • Trauma-informed and client-centered approach
  • Ability to manage high-volume inquiries
  • Cultural humility and confidentiality
  • Strong problem-solving and decision-making skills
  • Knowledge of homeless services systems and eligibility guidelines
  • Knowledge of HUD homeless assistance programs and fair housing principles
  • Strong organizational, communication, and documentation skills
  • Possess a valid California driver's license and have access to a properly registered vehicle.
  • Ability to pass a background check and criminal clearance
  • Ability to successfully pass a TB screening test.

Responsibilities

  • Conducts in-person, phone, and virtual intake screenings for individuals and families experiencing or at risk of homelessness
  • Determines eligibility for homeless prevention, emergency services, and housing programs in accordance with HUD and funder guidelines
  • Verifies income, housing status, household composition, and required documentation
  • Explains program requirements, participant rights, and responsibilities clearly to clients
  • Completes standardized assessments as required for coordinated entry systems
  • Prioritizes clients based on vulnerability, risk, and program eligibility
  • Facilitates referrals to appropriate homeless prevention, shelter, rental assistance, and supportive service programs
  • Coordinates warm handoffs to case managers, outreach staff, or partner agencies
  • Maintains accurate, complete, and timely client records in Exym and other required systems
  • Ensures compliance with confidentiality, data quality, and contractual documentation standards
  • Tracks intake outcomes, waitlists, and referrals
  • Provides trauma-informed, respectful, and culturally responsive customer service
  • Responds to inquiries from clients, partner agencies, and community members
  • Maintains professional boundaries and ethical standards
  • Supports program staff with intake coordination and scheduling
  • Participates in trainings, meetings, and quality improvement activities
  • Assists with monitoring and reporting as assigned
  • Assists with conducting street, shelter, and community-based outreach to individuals and families experiencing homelessness or at imminent risk of housing loss
  • Other duties as assigned by leadership
