FTE: 1.0_, Shift: M-F_, Schedule: 8-5 Position Summary The Intake Coordinator RN is an excellent opportunity for those seeking a meaningful and gratifying career in health care. You will play a vital role in our health care team by coordinating referrals from physicians, hospitals, facilities and other community referral sources. As an RN at MultiCare, you will apply specialized knowledge, critical thinking, and independent decision making skills daily to ensure the best possible health care experiences and outcomes for our patients and families.

Requirements

  • Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement
  • Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
  • Related clinical experience preferred
  • Home Health/Hospice specific: Minimum of two (2) years of Medicare Certified Home Health and or Hospice nursing experience

Nice To Haves

  • Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred

Responsibilities

  • You will receive and review referral information to ensure referrals are appropriate
  • You will engage your critical thinking skills in making decisions regarding patient care, including knowledge and integration of relevant standards, resources, and data
  • You will utilize the knowledge base of education and experience to deliver care and services according to hospital policies, procedures, and protocols
  • Collaborate across team to facilitate overall improvement in quality of care, patient experience, and cost reduction
  • Foster a learning environment for patients/families, nursing staff, and other members of the healthcare team
  • Advocate for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds
