About The Position

Summary: The Coordinator, Integrated Care (CIC) is the link between patients, community members and Tapestry 360 Health (Tap 360). The Integrated Coordinator partners with patients and community members to provide disease-specific health education and connect them with resources for identified social needs.

Requirements

• A minimum of two years of healthcare or public health experience is required.
• Experience with electronic medical record systems and Microsoft Office is required.
• High school diploma or GED Required

• Associate or bachelor’s degree is preferred.
• Bilingual and/or bicultural preferred.
• Top spoken languages in our service area: Spanish, Arabic, Vietnamese, Burmese, French, Amharic, Hindi.
• Excellent interpersonal communication skills.

• Familiarity with and/or ability to learn how to use Microsoft Office.

Responsibilities

• Provide peer support and advocacy for patients navigating the medical system and building relationships with family and other individuals
• Assesses health risk and social needs for a given population, and connects patients/participants to needed resources
• Participates in addressing care gap measures both for existing patients and encourages/educates community participants about these necessary tests/activities.
• Maintains and distributes approved disease-specific education
• Communicates with patients regarding scheduled initial appointments when needed. At times, may assist patients in obtaining primary care, specialty care, and ancillary services.
• Assists with patient referrals from community partners.
• Enters and maintains electronic medical records, compiles reports and completes other program documentation
• Supports population health and quality improvement efforts, including conducting outreach and education to targeted populations of patients
• Other duties may be assigned
