Clarvida is seeking an Intensive Case Manager to support individuals with significant behavioral health needs through community-based services. In this role, you will work closely with clients to implement an Individual Recovery Plan (IRP) based on their self-identified goals, strengths, service needs, barriers, support systems, and emotional triggers. You will provide skill-building, training, and service coordination while linking clients to community resources. Through regular community visits, you will monitor client progress and identify additional supports needed to reduce hospitalizations, incarcerations, and episodes of homelessness while increasing housing stability, employment readiness, and community engagement. This role involves collaboration with medical providers, residential programs, and other service partners to deliver wraparound support.
