Position Overview Provide comprehensive outpatient primary care services for adult patients Diagnose and manage acute and chronic conditions including diabetes, hypertension, COPD, and heart disease Perform preventive care, annual wellness visits, and health screenings Develop individualized treatment plans and coordinate multidisciplinary care Collaborate closely with primary care providers and specialty teams Utilize EMR for accurate and timely documentation Participate in quality improvement and value-based care initiatives Shared call coverage depending on team structure
Requirements
- Board Certified or Board Eligible in Internal Medicine
- Eligible for Connecticut State medical licensure
- Active or eligible DEA registration
- Strong clinical, communication, and patient-centered care skills
Responsibilities
