About The Position

The Investigator & Incident Coordinator is responsible for coordinating, investigating, and resolving reportable incidents across assigned programs. Reporting to the Director of Incident Management & Investigations (PA or Regional), this role ensures incidents are documented accurately, investigated thoroughly, and resolved in compliance with regulatory requirements, organizational policy, and best practices. This position plays a critical role in promoting a culture of safety, integrity, and continuous improvement. The Investigator collaborates with program staff, leadership, and external stakeholders to ensure appropriate response to allegations of abuse, neglect, exploitation, and other reportable events. The role also serves as a resource and mentor to program-level incident reporters, supporting staff training and high-quality documentation practices. The Pennsylvania-based role focuses primarily on PA programs and regulatory agencies, while the regional role supports a broader multi-state footprint.

Requirements

  • Bachelor’s degree preferred in Human Services, Criminal Justice, or a related field
  • High School Diploma with 10 years of incident management experience may substitute for degree
  • Certified Investigator credential required
  • Minimum 3 years conducting investigations in behavioral health or IDD settings
  • Familiarity with state-specific systems and agencies strongly preferred for PA-based role
  • Valid driver’s license with clean motor vehicle record and willingness to travel
  • Satisfactory background check required

Responsibilities

  • Receive, review, and manage reportable incidents in alignment with funder and regulatory requirements
  • Conduct thorough investigations in accordance with state and federal guidelines and organizational policies
  • Interview staff, individuals served, families, and witnesses using trauma-informed practices
  • Collaborate with program leadership and Human Resources on personnel matters related to investigations
  • Ensure timely completion and submission of reports and supporting documentation to oversight agencies
  • Participate in incident review committees and contribute to follow-up planning
  • Serve as a subject matter expert on incident documentation and regulatory reporting
  • Train and support staff on incident management systems and documentation standards
  • Monitor corrective action plans and ensure timely follow-through
  • Maintain strict confidentiality and professionalism throughout investigative processes
  • Serve as liaison to external regulatory bodies as assigned
  • Travel to program sites to conduct in-person investigations as needed
  • Perform other duties as assigned
