About The Position

Stellar Care and Services is hiring for a job consultant in our Supported Employment program in Denver. This position will be part time with the option to supplement some of those hours in the Supported Community Connections department to ensure full time hours are met. Requirements

  • Must have reliable transportation, a valid driver's license, and a willingness to transport clients in your vehicle.
  • Personal vehicle use.
  • Must be at least 18 years of age due to driving demands
  • Must be able to work evenings and weekend
  • Able to perform the physical and mental demands of the position
  • This is a driving position that will require the use of your personal vehicle to transport the clients we serve. The State of Colorado requires that any vehicle used for this purpose pass a detailed multi-point safety inspection through an ASE certified mechanic. Some of the items on this inspection include windshield, suspension, brakes, lights, seatbelts and all other safety features. If offered this position, Stellar Care will cover the cost of the inspection, however any repairs required are at the expense of the applicant and must be resolved within 30 days of hire. Vehicles 5 years or newer are exempt from this inspection.

Responsibilities

  • Consistently exercises individual judgment, makes decisions and solves problems.
  • Completes daily notes of individual’s progress and challenges, timesheet and goal tracking in OnTarget.
  • Works with Program Director to update and maintain program participant records.
  • Reports complaints or performance concerns; manages conflicts and disagreements in a constructive manner; responds to grievances; identifies and solves individual’s problems.
  • Represent Program Director, Director or designee at internal and external meetings.
  • Coaches real-life job skills to individuals with disabilities at their workplace while providing the needed supports to help them obtain and maintain meaningful employment in the community.
  • Ongoing communication with workplace supervisors, team members, and family members
  • Provides on-site training, instruction, supervision, and advocacy for individuals at community job sites
  • Maintains and distributes employment information such as job openings to Program Director and individuals.
  • Support individuals in increasing independence in their workplace and community
  • Assist individuals in understanding the functions and additional tasks of their job.
  • Assist individuals in communicating and cultivating meaningful relationships with coworkers and management within their job and professional community.
  • Assist individuals in advocating for their needs while at work, including potential formal accommodations needed.
  • Maintain proper community supervision requirements.
  • Transportation to and from work/interviews if required.
  • Provides medication administration to individuals served when needed under QMAP certification.
  • Proactively performs outreach to cultivate relationships with perspective employers.
  • Maintains a healthy relationship with individuals serving as their single point of contact as needed; checks in regularly with the individual throughout their employment and connects them any supports or resources needed through job placement.
  • Carries out duties as assigned by Program Director and/or designated supervisor.
  • Performs all other duties as assigned.
