Basic Function The Lead Autonomous Family Nurse Practitioner (FNP) serves as both a high-level clinical and a frontline clinical leader. This role provides comprehensive medical services – including assessing, diagnosing, prescribing, treating, and educating patients – while also leading a multidisciplinary clinical team. The Lead FNP will oversee and support a team including APP, RN, and MAs, providing mentorship, clinical guidance, and workflow leadership. Approximately 70% of the role is patient-facing, with the remaining time dedicated to leadership responsibilities, team development, and operational improvement. The clinic serves a diverse patient based encompassing a wide range of identities, life experiences, and health needs. While the predominant patient population is adults, this physician will serve patients of all ages. The clinic accepts all major insurance providers, ensuring broad access to care for patients.

Requirements

  • 5+ years of Family Nurse Practitioner experience
  • 2+ years of clinical leadership experience; demonstrated experience supervising clinical staff
  • Masters of Science in Nursing
  • Successful completion of Advanced Practitioner training and certification as a family practice Nurse Practitioner from an approved Nurse Practitioner program
  • Current state Registered Nurse license
  • Current state license as an Advanced Registered Nurse Practitioner with prescriptive authority
  • Strong communication skills and a commitment to patient-centered care
  • Comfort with managing complex medical and psychosocial needs
  • Clinically versatile; demonstrates strong proficiency in clinical skills with the ability to manage a wide range of acute and chronic conditions across age groups

Responsibilities

  • Assesses patient status by obtaining health and mental health history through patient/family interviews and chart reviews; assesses presenting illness, risk factors, family history, psychosocial situation, and cultural factors; and performing appropriate physical examination
  • Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness
  • Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks
  • Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws
  • Instructs patient/family regarding medications and treatments. Educates patients regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs
  • Communicates appropriate case management information to other professionals and community agencies
  • Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures
  • Provides day-to-day clinical leadership, fostering a culture of collaboration, accountability, and continuous improvement
  • Serves as a clinical resource and mentor, offering guidance on complex cases, clinical decision-making, and best practices; facilitates interdisciplinary communication and ensures alignment across the care team
  • Works to optimize workflows, patient flow, and care coordination; helps establish and refine clinical protocols, quality metrics, and care pathways that elevate patient outcomes and operational efficiency
  • Supports onboarding, training, and skill development for clinical team members
  • Contributes to shaping clinic culture, supporting a mission-driven, community-focused approach to primary care
