About The Position

"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care setting while supervising nursing assistants and ensuring high standards of patient care. The LPN will follow established healthcare guidelines and collaborate with physicians and care teams to support residents’ health and well-being.

Requirements

  • Graduate of an accredited Licensed Practical Nursing (LPN) program .
  • Active and unrestricted LPN license in Massachusetts .
  • BLS / CPR / AED certification required.
  • Strong knowledge of nursing practices, patient care procedures, and healthcare regulations.
  • Ability to work effectively with residents, families, and interdisciplinary care teams.
  • Strong documentation and clinical assessment skills.
  • Ability to lift up to 50 pounds and meet physical requirements of patient care.

Nice To Haves

  • Previous experience in long-term care or skilled nursing facilities preferred .

Responsibilities

  • Provide direct nursing care to residents in accordance with established care plans and healthcare regulations.
  • Supervise and direct the day-to-day activities of nursing assistants.
  • Administer medications and treatments as prescribed by physicians while verifying resident identity.
  • Monitor residents’ conditions and document care, treatments, and responses accurately in medical records.
  • Receive and transcribe physician orders into charts, medication records, and care plans.
  • Participate in admission, transfer, and discharge processes, ensuring accurate documentation.
  • Conduct routine patient assessments including vital signs, blood pressure, and TPR.
  • Collect specimens such as urine, blood, and sputum for laboratory testing.
  • Perform clinical procedures including catheterization, tube feeding, wound care, dressing changes, suctioning, and other nursing treatments as required.
  • Collaborate with physicians and interdisciplinary teams to support resident care plans.
  • Ensure adequate supplies of medications, equipment, and nursing materials are available on the unit.
  • Provide education and guidance to residents and family members regarding care and treatments.
  • Maintain strict confidentiality of patient information and comply with all healthcare privacy regulations.
  • Participate in facility inspections and regulatory surveys as required.
  • Support a positive care environment through compassionate and professional patient interactions.
