This position provides comprehensive, intensive case management and complex service coordination for youth and young adults identified as high-risk. High-risk individuals are defined as those who, without significant support, would likely be or have previously been hospitalized, institutionalized, incarcerated, subject to legal action, or pose an imminent danger to themselves or others. The Case Manager utilizes advanced assessment skills and in-depth knowledge of multiple interconnected systems to mitigate complex support needs, coordinate services, ensure client safety, and facilitate transitions to the least restrictive settings. This role requires a higher-than-standard frequency of contact and coordination with support teams regarding service delivery, risk mitigation, serious incidents, and protective service issues. This is a Limited Duration. A limited duration can last up to two (2) years, but could end at any time.
