This position provides comprehensive, intensive case management and complex service coordination for youth and young adults identified as high-risk. High-risk individuals are defined as those who, without significant support, would likely be or have previously been hospitalized, institutionalized, incarcerated, subject to legal action, or pose an imminent danger to themselves or others. The Case Manager utilizes advanced assessment skills and in-depth knowledge of multiple interconnected systems to mitigate complex support needs, coordinate services, ensure client safety, and facilitate transitions to the least restrictive settings. This role requires a higher-than-standard frequency of contact and coordination with support teams regarding service delivery, risk mitigation, serious incidents, and protective service issues. This is a Limited Duration. A limited duration can last up to two (2) years, but could end at any time. Requirements

Associate Degree from an accredited College or University or equivalent.
Four (4) years of case management experience with a minimum of one year of experience carrying a full case load.
Must have a valid driver's license
Must pass a criminal background check

Responsibilities

Conduct comprehensive, advanced assessments to determine the complex, and offer multi-system support needs of high-risk individuals.
Approve appropriate DD-funded services across multiple systems settings-including Intellectual/Developmental Disabilities (IDD), Behavioral Health, Child Welfare, Juvenile/Adult Justice (including Oregon Youth Authority), and Crisis services-to mitigate assessed risks and prevent danger to self/others and community members.
Monitor and document the use of behavior services, manual restraints, and psychotropic medications to ensure compliance with Department-approved behavior model, Positive Behavior Support (PBS), Oregon Intervention System (OIS) and OARs regarding psychotropic medications.
Approve and complete ODDS required documentation for appropriate adult services in foster care, group homes, and exceptional situations when a minor needs access to adult settings.
Write and implement Variances and Safety Assessments to effectively mitigate behavioral risks within community-licensed settings.
Facilitate complex client transitions, including when children turn 18 and assist them to access ‌adult services, when children require initial placement, and when youth transition back to their family homes.
Provide technical assistance to services coordinators (case manager 2 classification) and their supervisors with initial out-of-home placements, including referrals, screenings and documentation to move a child into a residential setting.
Monitor all services and supports (financial, medical, behavior, educational, transportation, etc.) through high-frequency home visits, service provider visits, service provider visits, and phone calls to ensure appropriateness and quality.
Ensure that all resources are used appropriately to meet the client’s developmental disabilities needs.
Provide information and notice regarding client rights and coordinate the resolution of grievances.
Facilitate the annual Service Coordinator ONA Review assessment with the consumer, family, guardian, and collateral contacts to create an accurate portrayal of the client’s strengths, needs, choices, and risks.
Coordinate protective services in high-stress situations, including behavioral escalations and initial Child Welfare involvement related to the abuse or neglect of a minor or vulnerable adult.
Develop, submit, and modify necessary documentation promptly to implement, change, or adjust payments and program benefits.
Independently identify statewide resources appropriate for individuals and work to secure needed resources in order to stabilize the situation of an individual in crisis.
Perform routine crisis management documentation, including the submission of Serious Incidents (SIS) and Death, Abuse, Neglect Referrals (DANs) in applicable databases, and review a high volume of incident reports.
Provide technical assistance and training to Individuals, families, care providers, and community partners on the implementation of Individuals Support Plans (ISP), service delivery protocols, and risk mitigation strategies.
