About the Home Health/Hospice Team Our Home Health/Hospice team prides itself on providing top quality care to our patients in their own home following hospital discharge, change in health condition, or transition to hospice services. Mission for Position The Home Health LPN provides compassionate, high-quality nursing care in the home and hospice setting by supporting individualized care plans, promoting safety and dignity, and collaborating with the interdisciplinary team to enhance the well-being of clients and their families.

Requirements

  • Graduate of an accredited school of nursing.
  • Current LPN licensure in the State of Minnesota.
  • Current, valid MN Driver’s License and current auto insurance.

  • Experience in home care/hospice preferred but not required.
  • Compassionate caregiver committed to patient dignity and comfort.
  • Strong communication and teamwork skills for collaboration with interdisciplinary care teams.
  • Organized and dependable with the ability to prioritize tasks and manage multiple client needs.
  • Skilled in observation, documentation, and reporting changes in patient condition.
  • Knowledge of infection control, safety practices, and clinical procedures.
  • Comfortable providing care in home and hospice environments with a patient- and family-centered approach.

Responsibilities

  • Provide direct nursing care to home care and hospice clients following the RN-directed plan of care.
  • Carry out physician orders and communicate changes in client condition to the RN and care team.
  • Document client care accurately and share relevant updates with interdisciplinary team members.
  • Maintain infection control, safety standards, and proper clinical procedures.
  • Support care coordination including scheduling, referrals, and client admissions as assigned.
  • Collaborate with team members to ensure clients receive consistent, high-quality care.
