About The Position

The LPN Nurse Navigator will assist the RN with the management of patient caseloads to ensure timely access to care and removal of barriers to treatment.

Requirements

  • A valid Licensed Practical Nurse multistate or Florida single-state licensure is required
  • Three (3) years of LPN experience
  • Strong organizational skills
  • Ability to prioritize and reprioritize quickly
  • Strong written communication skills
  • Strong telephonic communication skills
  • Ability to work autonomously and with a virtual team in a remote work environment
  • Proficient in Microsoft Word, Excel, and Outlook
  • Possess high-level critical-thinking skills

Responsibilities

  • Assist patient and family in the coordination of care and management of a patient's individual health needs throughout the care continuum.
  • Assist the RN with the management of patient caseloads by providing prompt response to telephone inquiries and other issues of a clinical nature as directed.
  • Address barriers to care for patient, caregiver, or family needs to achieve optimal patient outcomes.
  • Assist high risk patients with the goal of minimizing Emergency Department and inpatient readmission.
  • Provide patient-centered, individualized ongoing education, resources, and referrals to internal and external resources.
  • Works as an integral team player and is expected to adhere to and abide by the rules and regulations set forth by the Florida State Board of Nursing.
  • Additional tasks as needed to support the team
