About The Position

Acts as a navigator, working with ED Charge RN to place patients in correct care area, assist with transitions for admitted patients, discharged, and transferred patients. Ensures a high level of customer service and resolves issues promptly that may arise. Provide suggestions for service improvement. Maintains a comprehensive and collaborative relationship with patients, family, physician, and health care team to effectively navigate care transition needs. Familiarity with local support/advocacy and resource organizations that provide aid and services to patients. Maintains a collaborative relationship with case management, social workers, and CCT Team: refers any cases requiring consultation such as patient leaving before completion of treatment, patients in need of PCPs, and patients in need of additional education on available resources. Coordinates follow-up for transitions of care, including follow-up calls for discharged or transferred patients to ensure understanding of prescriptions and follow-up instructions. Procures services and serves as an advocate on behalf of patients and families for scarce resources. Actively participates in hospital care transition efforts and acts in a manner that promotes positive team relationships. Effectively communicates with core members of healthcare team to coordinate care transitions. Assist with all Adult and Child Protective Servies cases and provide follow up to Case Management Team. Work collaboratively with NGH Foundation in yearly organization of toiletry and clothing drives for patients in need. Understanding of health care disparity issues, and the ability to interact with patients and families of diverse cultural backgrounds. Ability to adapt to evolving goals and requirements and to test new approaches to improve patient care. Displays evidence of excellent leadership skills along with verbal, written, and interpersonal skills, demonstrates excellent communication skills. Displays critical thinking and clinical problem-solving skills. Ability to use independent judgement and compassion when carrying out tasks. Flexibility in work schedule to provide optimal coverage for patient needs. Facilitates a patient care process that increases the likelihood of achieving established outcomes and goals. Demonstrates flexibility and creativity in identifying resources to meet patient care needs. Assesses patient’s understanding of current healthcare issues and reinforces information about plan of care as needed. Participates in research and quality improvement initiatives to continuously evaluate the effectiveness of interventions, and their impact on clinical outcomes and patient satisfaction. Initiates and contributes to modifications and/or changes in practice to achieve high quality of care, patient satisfaction, and appropriate use of resources. Participates in departmental and organizational committees. Perform other duties as required.

Requirements

  • Graduate of an approved Nursing Program
  • Active Tennessee Nursing License
  • BLS required before start date.
  • ACLS, PALS, TNCC, and CPI required within 90 days of hire.
  • 2 years Emergency Department experience required.
