About The Position

This position is responsible for providing education and support to screening and patients. Responsibilities include addressing healthcare disparities and barriers to care for patients; including facilitation of communication across the continuum of care, coordination of resources, education and advocacy, and coordination of care with managing physicians and the interdisciplinary team. May participate in some aspects of clinical care. This position exists in various departments throughout OhioHealth system of hospitals and healthcare services. The nurse navigator position supports our growing Movement Disorders and Epilepsy programs at OhioHealth. Our nurse navigator works directly with patients by helping navigate them through our clinical pathways, including clinical visits and testing at OhioHealth. This position is part of a larger team and is integral to the planning and success of patient outcomes that allow our physicians to make good clinical judgments on appropriateness for surgery. Our navigation pathways take roughly 2-3 months from initial evaluation to surgical case conference, during which patients undergo specific tests and visits designed to enhance clinical decision making around whether candidates are appropriate for surgery. The nurse navigator tracks and prepares patient presentations for interdisciplinary case conferences. Specific tasks include answering patient questions about the pathway and surgeries, ensuring testing is completed in a timely manner, coordinating and bridging gaps in care, and providing focused patient care, therapeutic communication, and basic outcomes and data tracking.

Requirements

  • Bachelor's Degree (Required)
  • RN - Registered Nurse - Ohio Board of Nursing

  • Ideal candidate will be interested in this area or Neuroscience in general, excellent communicator, and a strong work ethic.
  • Prior nurse navigator work is desirable.

Responsibilities

  • Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to patients and families.
  • Participates in quality control and assurance.
  • Participates in process improvement activities.
  • May participate in some aspects of clinical care as needed.
  • Other duties may vary by department such as various program coordination and maintaining registries.
