We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home. We offer an array of medical and chronic care services and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years. We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home. Homebase Medical is growing in the Central San Diego / Chula Vista area. We are currently searching for an experienced Nurse Practitioner or Physician Assistant to engage in Medicare Wellness Visits at our contracted partner clinics and the occasional home visit. This role requires fluency in Spanish. This is a full-time position of 5 days per week, 8-hour days (8am - 5pm / 40 hours per week)

Requirements

  • Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
  • Current, unrestricted NP or PA license to practice in California, including DEA license.
  • Minimum of 2 years of experience in primary care, hospice, or urgent care/outpatient setting.
  • Minimum of 1 year of previous experience utilizing an EMR system.
  • Spanish speaking mandatory
  • Good working knowledge of HCC coding.
  • Strong computer skills including MS Office and Outlook is strongly desired.
  • Great verbal and written communication skills
  • Current drivers’ license, auto insurance and reliable transportation

Responsibilities

  • Perform Wellness Visits in partner primary care clinics, facilities and in home within the Chula Vista community.
  • Drive to designated facility locations in your assigned geographic area.
  • Review patient medical and family history.
  • Review of current providers and prescriptions.
  • Perform height, weight, blood pressure, and other routine measurements.
  • Deliver personalized health advice.
  • Review risk factors and potential treatment options.
  • Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
  • Provide health education to patients.
  • Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
  • Provide referrals to appropriate medical resources.
  • Utilize our EMR system to document patient visit/interaction accurately and timely.
  • Knowledge of HCC coding.
  • Other duties as assigned.
