All Nurse Practitioners, Primary Care work collaboratively with Signature Health staff, including the behavioral health providers, counselors, case managers, and outside partners to achieve holistic, integrated care.

Requirements

  • Ohio Licensed Advance Practice Nurse.
  • Graduation from an accredited university with a background in science, including a BA or BS in Biology, Chemistry, Nursing, Anatomy and Physiology, Public Health, Behavioral Science, or a similar degree.
  • Certified through the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners Certification Board (AANPCB)
  • Current DEA registration.
  • American Heart Association (AHA) Basic Life Support (BLS) certification required
  • Proficient in Microsoft Office and Electronic Health Records systems with demonstrated ability to learn new platforms.
  • All employees of Signature Health are required to comply with the Signature Health Annual Influenza Vaccination Policy. This policy requires employees to obtain an annual flu vaccination. A medical and/or religious exemption may be submitted for review by the Signature Health Review Committee. Exemption requests are not guaranteed to be approved. Responsibilities

  • Obtaining and legibly recording, a detailed medical history of each patient, including, but not limited to, the patient’s chief complaint, history of present illness, and past medical, social, and family history, all as medically appropriate.
  • Demonstrated clinical competence including physical assessment, health teaching and counseling, and make sound clinical judgments based on assessment of the physical, psychological, social and environmental needs of the client.
  • Prescribe treatment for illnesses within the Nurse Practitioner’s scope of practice, based on standing orders and nurse practitioner formulary.
  • Determine which problems are within scope of practice, and which require referral to a physician according to mutually accepted guidelines.
  • Analyze health behavior related to personality, life style, socio-economic status and culture.
  • Analyzing reports and/or findings of tests and personal examinations in order to assess the medical condition of the patient, including diagnosis and prognosis.
  • Referring the patient to other services within the Signature Health, other physicians, including specialists and other health care providers, when and if needed.
  • Prescribing medication and treatment for the patient that is consistent with standard clinical practice, and the APRN’s education, training, and scope of practice.
  • Performing such clinical procedures and therapeutic techniques as are consistent with APRN’s prior practice patterns and training.
  • Preparing and attending to all reports, claims, and correspondence necessary or appropriate to the performance of the professional services.
  • Attending professional conventions and post-graduate seminars as well as participating in professional societies on a reasonable basis.
  • Promoting the professional practice of Signature Health.
  • Providing coverage when other physician/APRN employees/contractors are unavailable due to vacations, holidays, illness, and attendance at professional activities.
  • Timely reporting of major unusual incidents and other information required to appropriate governmental authorities and as required for Signature Health reporting.
  • Participate in Signature Health quality assurance and peer review activities.
  • Performing other duties assigned by Signature Health from time to time that are commensurate with professional services normally and customarily performed by an APRN.
  • Being available to give coverage for other providers and taking client calls as assigned by Signature Health.
  • Following all policies and procedures of Signature Health.
