About The Position

PIVOT Rehabilitation is hiring a Licensed Home Health Certified Occupational Therapy Assistant (COTA) for a PRN home health occupational therapy assistant job providing in-home occupational therapy services to adult and senior patients in Dallas \ Fort Worth, TX and surrounding DFW areas. Requirements

  • Associate Degree from an accredited Occupational Therapy Assistant program
  • Active Texas Certified Occupational Therapy Assistant (COTA) license
  • Current CPR certification
  • Strong communication, clinical judgment, and time-management skills
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to travel throughout Dallas and the surrounding DFW areas

Nice To Haves

  • Home health occupational therapy experience preferred (1+ year preferred)
  • Knowledge of Medicare home health documentation standards preferred

Responsibilities

  • Provide skilled home health occupational therapy interventions under the supervision of a licensed Occupational Therapist (OT)
  • Implement and carry out individualized Plans of Care (POC) developed by the supervising OT
  • Provide treatment for orthopedic, neurological, cognitive, and post-surgical conditions
  • Address ADLs and IADLs, including bathing, dressing, grooming, toileting, feeding, and home management tasks
  • Facilitate improvements in upper extremity strength, coordination, ROM, balance, and functional mobility
  • Implement fall prevention strategies and home safety interventions
  • Train patients and caregivers on adaptive equipment, assistive devices, and durable medical equipment (DME)
  • Recommend environmental modifications to improve safety and independence
  • Complete accurate and timely documentation in the EMR
  • Communicate effectively with the supervising OT, physicians, and interdisciplinary team members
  • Follow all home health regulations, infection control standards, and safety guidelines
  • Participate in QAPI and agency performance improvement initiatives
