We’re currently seeking an Occupational Therapist to join a skilled nursing facility in Sacramento, CA. Sacramento offers a great mix of city energy and laid-back California living, with plenty of neighborhoods, parks, and local spots to explore. Requirements

  • Active Occupational Therapist license in the state of California.
  • Master’s or Doctorate degree in Occupational Therapy from an accredited program.
  • Knowledge of post-acute rehabilitation, ADL retraining, and adaptive equipment.
  • Ability to assess, develop, and implement individualized treatment plans.
  • Strong communication and interdisciplinary collaboration skills.
  • Proficient documentation and time management skills.

Responsibilities

  • Evaluate residents and develop individualized treatment plans.
  • Provide skilled OT services to improve ADLs and functional mobility.
  • Implement interventions for orthopedic, neurological, and post-acute conditions.
  • Train residents in adaptive techniques and use of assistive equipment.
  • Collaborate with interdisciplinary team to coordinate care.
  • Educate residents and families on treatment plans and discharge goals.
  • Document evaluations, daily notes, and progress reports timely.
  • Monitor progress and adjust treatment plans as needed.
  • Ensure compliance with facility policies and state regulations.
