We’re currently seeking an Occupational Therapist to join a skilled nursing facility in Sacramento, CA. Requirements
- Active Occupational Therapist license in the state of California.
- Master’s or Doctorate degree in Occupational Therapy from an accredited program.
- Knowledge of post-acute rehabilitation, ADL retraining, and adaptive equipment.
- Ability to assess, develop, and implement individualized treatment plans.
- Strong communication and interdisciplinary collaboration skills.
- Proficient documentation and time management skills.
Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide skilled OT services to improve ADLs and functional mobility.
- Implement interventions for orthopedic, neurological, and post-acute conditions.
- Train residents in adaptive techniques and use of assistive equipment.
- Collaborate with interdisciplinary team to coordinate care.
- Educate residents and families on treatment plans and discharge goals.
- Document evaluations, daily notes, and progress reports timely.
- Monitor progress and adjust treatment plans as needed.
- Ensure compliance with facility policies and state regulations.
