About The Position

Sun River Health provides the highest quality comprehensive primary, preventive, and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-For-Profit Health Center serving communities in Suffolk County. We currently have the need for a full-time Peer Outreach Worker to join our team located in Goshen, NY. Under the supervision of the Senior Director, the Peer Outreach Worker will work with the Assistant Director and Senior Prevention Counselor to conduct culturally sensitive, alternative venue outreach to engage in low threshold services for young men of color who have sex with men (YMCSM) and young people of transgender experience as well as youth at risk of HIV, STIs, medical, homelessness, public assistance, housing issues, Insurance, etc.. Engagement will be completed in person on the Mobile Medical Unit (MMU) as well as at the THRIVE by Safe Space and other events to provide low threshold services. There will be follow-up on the phone and through the internet. The candidate will also maintain the medical lab logs and outreach MMU logs as it relates to new venues through outreach to find new locations. Essential Functions: Collects biopsychosocial assessment data. Participates in outreach/field events as scheduled. Provide follow up interventions including phone calls and escorts to participants in need of case management and supportive services. Maintain up to date client records in accordance with grant and HIPPA protocols. Promote YAP program through various outlets including internet/social media, in-reach and out-reach activities. Attends and participates in continuing educational programs, seminars, etc., designated to maintain professional competence. Provide testing on a daily basis as clients must complete DOH HIV testing training. Work in-kind as correlates with the Thrive by Safe Space and field events.
