Lurie Children's Hospital of Chicago Job Description The Patient Navigator manages a variety of office functions and is key to smooth operations of the Division. The primary function of this position will be to coordinate health care services, in partnership with patients and care team members, across the continuum of care in order to maximize and improve patient health outcomes. The core functions of a Patient Navigator includes: assisting patients and families with understanding and adhering to treatment plans; assisting patients in accessing health services; ensuring barriers to obtaining care and/or social services are addressed; linking patients and families to community agencies and services; and collaborating with the care team, regarding transitions to care.
