About The Position

Adventist Health Ukiah Valley has been deeply connected to the community we
serve since 1956. We are comprised of a 50-bed acute care medical center, level
IV trauma center, level II intensive care nursery services, extensive physical
rehabilitation program, outpatient surgery and diagnostic center and advanced

Job Summary:

Formulates and executes medication management systems and pharmacotherapy
services. Ensures that medications, supplies, and drug information are furnished
to the customer. Collaborates with dedicated team members across disciplines to
create original policy and models to promote appropriate and effective
medication utilization and harmonious application across all disciplines.
Monitors drug therapies, identifies and troubleshoots potential medication
safety hazards and documents all interactions for medical review. Assists in
making recommendations on programs, process improvement or development of new
pharmacy services.

Requirements

• Graduate of school of pharmacy: Required
• Doctorate of Pharmacy (PharmD): Required
• Pharmacist Intern License or Registered Pharmacist license in the state of practice: Required (May be pending, but must be duly licensed by December 31st of the practice year unless otherwise approved by accreditation agency)
• All licensures and certifications needed to fulfill the duties required of a Pharmacist at the state level: Required
• Pharmacist Intern License or Registered Pharmacist license in the state of practice : Required
• ACLS (Advanced Cardiovascular Life Support): Required
• Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required
• AHA-PALS: Required

Nice To Haves

• One year's technical experience: Preferred
• Inpatient clinical pharmacy experience: Preferred

Responsibilities

• Processes, enters, verifies, and sends medication orders within the institution in a timely manner that is in compliance with department and hospital policies, Joint Commission and other regulatory guidelines, while actively ensuring that medication therapy is efficacious, safe, convenient, and cost effective and establishing the best therapeutic outcomes. Assures that medication prescribing is optimal with respect to indication, dose, route, frequency, history of allergy, and does not duplicate or interact with the effects of other drugs.
• Conducts and manages daily reports on medication utilization based upon daily rounds to identify, catalogue, and subsequently analyze areas in which medication improvement is needed.
• Participates in all pharmacy-related tasks and programs such as clinical rounds, emergency department, short-staffed areas. Expedite changes needed to meet regulatory, accrediting, licensing, and government agency standards and laws.
• Communicates recommendations to patients and health care providers who provide care directly to patients. Designs, conducts, monitors and evaluates the outcomes of organized and structured programs. Develops and implements medication-use policy.
• Individuals experiencing exposed rashes, sunburn, weeping sores, conjunctivitis, active respiratory infections or other communicable disease, or those wearing cosmetics, nail polish, or artificial nails shall be excluded from the ISO Class 5 and ISO Class 7 compounding areas until their conditions are remedied.
• Performs other job-related duties as assigned.
