About The Position

The Pharmacist is responsible for providing pharmaceutical services in accordance with policies and standard operating procedures set by the Corporation and within the constraints of the South Carolina Board of Pharmacy act. The pharmacist performs duties under the direct supervision of the Pharmacist-In-Charge and the Pharmacy Services Director and is responsible for preparing and dispensing medications, ordering, receiving and storing pharmaceutical supplies; maintaining drug utilization records, consulting with health care professionals (providers/ancillary) and analyzing patient drug profiles.

Requirements

  • A PharmD or Bachelor of Science degree in Pharmacy and possess current license in good standing to practice pharmacy in the State of South Carolina.
  • Current Basic Life Savings (BLS) Certification.
  • Must have thorough knowledge and ability to utilize a computerized pharmacy system.
  • Must be flexible; capable of performing a variety of duties.
  • Must be able to meet and communicate effectively with the general public.

Nice To Haves

  • Prefer completed ACPE accredited Pharmacy-Based Immunization training program.
  • Prefer a minimum of three (3) years experience as a pharmacist.

Responsibilities

  • Maintains all aspects of New Horizon Family Health Services, Inc. pharmacy program in strict accordance with Federal and State regulations.
  • Adhere to 340B program administration requirements.
  • Prepares medications by reviewing and interpreting prescriber’s orders; detecting therapeutic incompatibilities.
  • Dispenses medications by compounding, packaging and labeling pharmaceuticals.
  • Provides immunizations according to the most current SC Board of Pharmacy vaccine protocol.
  • Maintains current and accurate patient drug files.
  • Monitor drug therapies and advise prescribers of interventions
  • Control pharmacy operational requirements by organizing and directing pharmacy technicians work flow; verifying their preparation and labeling of pharmaceuticals; verifying order entries and charges
  • Performs medication consultation with each patient at the point of dispensation; informs the patient of the appropriate manner in which the medication is to be taken/used; conducts patient education pursuant to OBRA regulations.
  • Maintains adequate pharmaceutical inventory and monitor perpetual inventory on a regular basis.
  • Monitors stock for outdated drugs and disposes of them in strict accordance with established guidelines.
  • Provides pharmaceutical consultation to the medical staff.
  • Maintains access to online or written pharmaceutical information, i.e. Online Facts and Comparisons, FDA Orange Book
  • Assists in the preparation of the pharmacy for inspection by funding agency, Department of Health and Environmental Control, SC Board of Pharmacy and assists in the maintenance of an orderly pharmacy department.
  • Ensures that funds and supplies are utilized efficiently and effectively.
  • Must hold all patient Protected Health Information (PHI) other patient personal information and agency information in confidence, in accordance with the attached Employee Confidentiality Statement, which I have read, understand and signed.
  • Actively participate in and comply with all aspects of the NHFHS Corporate compliance Program, follow the Program Code of Conduct and obey all relevant laws, statutes, regulations and requirements applicable to Medicaid, Medicare and other State and Federal health care programs.
  • Observes schedule for work, lunch and breaks.
  • Participates in quality assurance and continuous quality improvement activities of the organization.
  • Performs other related duties which may be assigned by the Pharmacist-In-Charge, Pharmacy Services Director, Chief Medical Officer and/or Chief Executive Officer.
  • Acts as Pharmacist-In-Charge of pharmacy.
  • Mentors, trains and develops a high functioning pharmacy team, providing opportunities and challenging assignments to all pharmacy staff, to grow and develop, tailoring to meet each one’s needs.
  • Manages performance of pharmacy staff including scheduling and assigning tasks to ensure complex operational activities are met in a timely and efficient manner.
  • Proactively advances pharmacy initiatives including Medication Therapy Management (MTM), immunizations.
  • Maintains knowledge and expertise of pharmacy technology systems. i.e. QS/1, POS, IVR, RxMedic, RxSafe/PakMyMeds in conjunction with the Pharmacy Services Director and Information Technology department.
  • Pre-book flu vaccine for all sites when appropriate.
  • Maintains a monthly inventory of all clinically administered 340B drugs.
  • Schedules all pharmacy department staff, process time off requests and process payroll.
  • Provides functional supervision to all pharmacy department staff.
  • Provides constructive feedback on all pharmacy staff evaluations.
  • Prepares reports as required by Pharmacy Services Director.
