Requirements
- Active, unencumbered Florida Physical Therapist license
- OASIS documentation experience required
- CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Professional liability insurance
- Workers’ Comp Exemption or Occupational Accident coverage
Nice To Haves
- Lymphedema certification preferred
Responsibilities
- Perform in-home physical therapy evaluations and treatments
- Develop and implement individualized care plans
- Complete Medicare OASIS documentation, including: Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges
- Evaluations
- Educate patients and caregivers on mobility, safety, and recovery
- Collaborate with physicians and interdisciplinary care teams
- Supervise Physical Therapy Assistants (PTAs) when applicable
- Complete documentation using AI-assisted EMR tools
