Requirements
- Licensed Physical Therapist in Pennsylvania
- Minimum of 1 year recent experience required, Home Health experience preferred
- Must have a reliable vehicle, valid PA driver’s license, and insurance coverage
- Must pass a satisfactory criminal record check

Responsibilities
- Assess and evaluate patients' physical conditions and needs to develop individualized care plans.
- Provide hands-on physical therapy treatments, including exercises and modalities, to enhance patient recovery.
- Document patient progress and maintain accurate records using EMR systems.
- Educate patients and families on treatment plans and home care strategies to promote independence and recovery.
- Drive to various locations for home health visits, ensuring timely and effective care delivery.
