The Population Health Manager leads and directs assigned staff to provide expert support to the various Population Health project teams and relevant Kaweah Health departments. Responsible for supporting, coordinating and facilitating the development of short and long-term work plans, monitoring progress towards goals, and evaluating performance of the Kaweah Health’s Population Health Projects. While key stakeholders from various Kaweah Health departments design and manage the implementation of Population Health programs, the Population Health Manager is responsible for the collection, organization and preparation of materials, assuring accuracy, timeliness and a coordinated approach to all Population Health projects. Establishes systems and standards for reporting to California Department of Health Services, maximizing current systems and providing vision and direction for future system development. The Population Health Manager reports to the Director of Population Health Management and works closely and effectively with the various project leads to achieve Population Health goals. Support with Medicare Population Health Management efforts, both Medicare Advantage (MA) and Medicare Shared Savings Program (MSSP). Provide Strategic guidance to Medicare programs. Review metrics, identify areas of risk for improvement, identify causal relationships and develop improvement plans. Requirements

  • CA RN license
  • Kaweah Health approved crisis intervention training within 60 days of hire/transfer
  • Bachelor’s degree
  • Two years of project management experience demonstrating the ability to lead and manage complex projects through completion
  • Strong report and/or grant writing experience
  • Significant knowledge of project management processes and related documentation practices.
  • Strong data analysis skills.
  • Strong interest in population health and public health.
  • Exhibits the ability to organize work assignments and follow through with accuracy.
  • Exercises good judgment, demonstrates initiative, emotional stability, tact and poise.
  • Demonstrated proficiency in the use of technology and software.

Nice To Haves

  • Certification/training in project management/business/healthcare administration (i.e. Project Management Professional (PMP), Lean Six-Sigma, etc.)
  • Certified Rural Health Clinic Professional (CRHCP) Certification
  • Master’s degree (preferably MSN, MPH or MBA)
  • Project management experience preferably in healthcare setting
  • Clinical experience
  • Experience with case management, chronic disease management of complex, high cost populations
  • Experience conducting health plan audits, specifically for CalAIM ECM, Chronic Care Management (CCM)
  • Experience with quality improvement, specific to the ambulatory care space and Rural Health Clinics (RHCs)
  • Deep understanding of HEDIS, Quality Measures, DHCS CalAIM Enhanced Care Management and Community Supports programs
  • Expert demonstrated knowledge of California Medicaid 1115 Waiver, PRIME/Delivery System Reform Incentive and Quality Incentive Pool (QIP) Program preferred.

Responsibilities

  • Oversees required clinical care plan assessment and implementation for patients enrolled in Population Health Programs (CalAIM ECM, CCM, etc.).
  • Leads and coordinates tasks towards successful completion of Population Health metrics and milestones.
  • Supports the organization and implementation of initiatives related to Population Health projects.
  • Ensures that all stakeholders understand the scope of the project as well as individual and departmental responsibilities and deadlines.
  • Organizes and leads committees and workgroups efficiently to support Population Health projects and achieve desired outcomes.
  • Identifies data requirements and specifications for required reporting and coordinates ongoing data collection.
  • Extracts, analyzes, evaluates and reviews data, operational documents such as program or financial reports.
  • Routinely monitors progress and prepares and presents project status reports to all stakeholders, including senior leadership when requested.
  • Collects and monitors data to assess performance to ensure milestones and metrics will be met.
  • Interfaces regularly and effectively with Project Owners to understand how best to support their needs, develop work plans, monitor progress toward goals and identify and resolve issues.
  • Identifies risk points and proposes solutions to problem areas; works with Project Owners and the Director of Population Health Management to appropriately escalate issues that jeopardize the success of projects.
  • Coordinates with subject matter experts to support Population Health project development and implementation.
  • Collects, organizes, reviews, and prepares documentation for all reports required by various agencies, to include audits by contracted health plans for CalAIM ECM.
  • Ensures all reporting requirements are met, completed and submitted on time.
  • Supervises and directs the activities of dedicated Population Health RN Case Managers and assigned support staff, Community Care Coordinators, Gaps in Care Team to ensure that patient care needs, data collection and reporting needs are met.
  • Works closely with information systems services department to ensure electronic health record development supports the needs of assigned projects (i.e. CalAIM ECM, CS, CCM, patient registries, patient portal, tele-health, etc.).
  • Coordinates activities with vendors and consultants and maintains positive working relationships with them.
  • Maintains records, files, and statistics pertinent to the job function.
  • Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
  • Performs other duties as assigned.
