About The Position

We are Hiring! Job Posting: PREVENTIVE CASE WORKER Location: Rochester, NY Department: CYF Employment Type: Full time Schedule: Monday thru Friday, 9am to 5pm, depending on the nature of the case Salary: $23.50 to $24.50/hr, depending on experience possibly more General Description Under general supervision, provides services, support and treatment to clients. Maintain confidentiality and ethical standards as defined by relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff, and is responsible for ensuring a trauma informed care environment. Essential Duties and Responsibilities Provides services to clients and their families primarily thorough face-to-face interactions. Services include but are not limited to counseling, training/education, advocacy, home studies, life skills, and completion of documentation required to support client situation. Completes assessments, formulates, implements, and monitors service delivery plans appropriate for client population supported by the program. Coordinate service delivery and complete required evaluations and screenings to include but not limited to medical, educational, legal, law enforcement, vocational and mental health. Facilitate and encourage connection with appropriate cultural and community resources. Attend appointments to include those required by various Courts. May be required to provide transportation to client and family members attending appointments. Work collaboratively with Monroe County Department of Human Services and other service providers. Assist clients with crisis such as lack of food, housing, abuse, domestic violence, etc. May be required to represent program at county level client service plan meetings. Attend and participate in local community events. Maintains knowledge of community resources available to client population. Meets regularly with supervisor to evaluate and implement professional growth in practice and performance. Inform supervisor in a timely fashion about significant events related to client issues. Participate in required Agency/Department/Program meetings, ongoing staff trainings, and program development. Initiate professional development through attendance at outside conferences, in-service training, reading and applying current, relevant literature, etc. Completes required record documentation in accordance with Program Policies and Procedures Maintain documented client case records that include all significant client interventions. Timely preparation and submission of required reports and statistics. Completes required record documentation in accordance with the program’s policies and procedures. Participates in relevant Agency meetings and/or trainings Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops. Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse Other duties as assigned Note: The above description is illustrative of tasks and responsibilities. Requirements

  • Education: Bachelor’s Degree in Social Work, Child Welfare or similar experience preferred. Equivalent combination of education and experience will be considered.
  • Credentials: Valid and clean NYS Driver’s License and reliable transportation.
  • Experience: Two or more years of experience in the child welfare or community service fields preferred.
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction
  • An ability to work efficiently and effectively and meet deadlines
  • An ability to work under pressure
  • Excellent verbal/written skills
  • Ability to maintain confidentiality
  • Ability to demonstrate behaviors and attitudes which support agency mission, philosophy, and policies.
  • Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
  • Possession of a valid NYS Driver’s license and use of a registered and reliable vehicle.
